📝 Need a Leave of Absence? Here’s How to Start

If you’re planning to be away for more than 3 consecutive days—whether it’s for medical reasons or unpaid leave—the first step is simple:

👉 Fill out this Request for Leave of Absence / FMLA Notice to submit your leave request. Or scan the QR Code below to fill out the form.

Once submitted:

  • The HR Department will be notified and will contact you with next steps.
  • If your leave may qualify for FMLA, we’ll review your eligibility.
  • If your leave is non-medical, we’ll guide you through the unpaid leave process.

If Your Leave is Medical and Qualifies for FMLA:

You’ll receive:

  • ISD 518 FMLA Policy (#410)
  • FMLA Poster
  • Job Description
  • Notice of Rights & Responsibilities
  • Certification of Health Form (to be completed by your healthcare provider and returned within 15 days—HR can fax it for you if needed)

After your certification form is received, HR will send you the final designation notice.

⚠️ Reminder: Employees must also enter their absences into the Red Rover system.
